Unique Identifier : AIDSLINE ICA10/94370504 AB OBJECTIVE: To develop and test intervention strategies with Men who have Sex with Men in Madras to reduce risk of HIV transmission. METHODS: Combination of qualitative and ethnographic research methodology. Concepts, identities and lifestyles, sexual behaviors, settings and practices obtained. Quanti fied circuits of MSM and of core groups at highest risk of HIV infection. The latter groups are typified by a high number of sexual partners and/or a high incidence of high risk sexual practices such as unprotected anal intercourse. These core groups and the context and setting in which they operate are the main targets of interve ntion to promote safer sex. Uses Enabling and persuasive approaches and focuses on outreach at cruising areas and key Brokers to reach circuit of Commercial boys. RESULTS: Different core groups of MSM at high risk of HIV infection were identified; Strategies designed to promote safer sex and a plan on how to reduce risk practices is being tested. Targets 1,400 Men (Danga and Double Deckers) cruising in Madras at various pick up points and Panthis (active partners). Most sexual practices involve oral sex. However it is estimated that 30% of these practices involve anal intercourse (Danga being the receptive partner). Condoms are made available close to where sex takes place. Skills in condom use being developed. The commercial sex workers are targeted through brokers who are being trained as change agents and condom sales agents. Panthi men who have sex with Danga on a very regular basis and have a preference for being inserters in anal intercourse are targeted with IEC on HIV/AIDS and need for condom use. STD prevalence in circuit is high. STD treatment center integrated with general health that is non stigmatizing towards MSM and acceptable to them started. Counseling and community support center started. CONCLUSIONS: Strategies of intervention in MSM circuits where men do not identify themselves as Gay/Homosexual in a homosocial setting being explored.
